steps have been taken prior to receiving this form:

2) They must have expressed a genuine interest in our services.
3) They must have given you permission for us to contact them.
We will pay referral fees to the referring party
after we have contracted with their prospect for our services.
In the event that the same prospective client
is submitted by more than one referring party, the first party
to submit this form will receive credit for the referral.

on additional services.
